The large flap-tear
A gentleman in his 50s walks in with a complaint of floaters, for the past 1 month. He had been ignoring it for quite some time but finally decided to consult. What I was wasn’t expecting to see…
RETINAL TEARS

….was this large ragged flap tear (can’t call it a horse-shoe exactly as it was more of a linear flap than semilunar!) in the inferior retina alongwith a mild vitreous hemorrhage. Prompt laser was undertaken to prevent a retinal detachment.
10 days later his vision was maintained at 6/6 with floaters subsiding, the tear was well barraged and a surprising finding can be seen. The flap had torn into two, one floating above the underlying retina and the other now free from vitreous traction.
Timely laser saved the day! And also led to a flap tear converting into an operculated hole (avulsed flap tear).
